NEW PATIENTS:
We require a $75 deposit (applied to your first visit) to hold a new patient appointment. This payment must be made via our online booking system within 24 hours of your appointment request in order to secure your appointment. When booking online, it will ask you for a card to be placed on file.
Please keep in mind, if you do not show for your appointment or cancel/reschedule with less than 24 hours notice, you will not be refunded for the deposit.
We utilizes a paperless health intake form which is sent upon acceptance of appointments scheduled via our online system. New patients who do not complete their intake forms 72 hours prior to their visit are at risk of having their appointment cancelled. We will always call and/or text you to notify of the cancellation.
For patients using insurance:
Please text a photo of the front and back of your insurance card plus your date of birth to our office at 208-391-3732. Once coverage is confirmed, we can schedule you via call or text.
